Grant Award Recipients

Signed signature page of the Grant Award letter no later than thirty (30) days from the date of your Grant Award letter.
This signed letter correctly sets forth your understanding of the terms of this grant, and if your organization agrees to be bound by these conditions, please indicate your agreement to such terms by submitting a copy of this letter countersigned by an authorized officer of your organization
Financial statement of budget vs. actual expenses of awarded grant funds within sixty (60) days following the end of the program/project for which funds were granted.
This report must provide comparing budget and actual expenditures of the grant, including a list of all vendors and amounts paid, and an evaluation of each program's statistics and expenses, assessing the success of the project vs. the Grantee's original objectives.
